Check with HR. Most likely, you'll be asked to complete the
form, and submit it to HR along with documentation of your B.A.
degree, credential, and subject matter competence.

No. In most districts once you have a certificate of compliance
you do not need to renew it. You only need to renew your
credential. The NCLB compliance remains vaild.
